Initial Evaluation (60 minutes) - $160

Individual Session (55 minutes) - $150

Individual Session (40 minutes) - $110

Insurance

Empowered Path is In network with Blue Cross Blue Shield (BCBS). 

 

For all other insurances, Empowered Path is considered an out of network provider. This means that we can provide a superbill following services to assist with obtaining out of network reimbursement from your insurance company. We encourage you to call your insurance company prior to booking to ask about your policy's out of network reimbursement benefits.

Steps to inquire about out-of-network benefits:

1. Contact your insurance provider on back on insurance card

2. Ask if your plan has out of network benefits for mental health

3. If asked, provide CPT codes "90791" and "90837"

4. If your plan has out-of-network benefits for specific CPT codes, ask what % is reimbursed and how to submit superbills to obtain reimbursement amount fastest.

Empowered Path is not responsible for fees not covered by your out-of-network benefits and you will be responsible for balance of session at start of session.

Sliding Scale

Sliding scale fees are available on a case-by-case basis. Please email us at billing@empoweredpathpllc.com to inquire about this option.

Missed Appointment and Cancellation Policy: 

If appointment is cancelled with less than 24 hour notice, you will be charged $30 to the card on file.
